Passport Service

When Can I Apply?

Hours by Appointment Only. Please call the Adult Reference Desk at 631-363-6133 ext. 4 or submit this form to make an appointment.

Please Note: The library does not process passport renewals, but we can review your application.

What Do I Need?

For full passport application details, visit the State Department’s website, travel.state.gov. You can also call the National Passport Information Center at 1-877-487-2778.

  • Application: Application DS-11 may be filled out online and then printed single-sided. The library can provide a paper application which must be filled out in black ink with no whiteout or cross outs. Do not sign the application until asked to do so by an agent.
  • Applicants and relevant parties: The applicant must be present. Both parents and children must be present for all minor applications under the age of 16.
  • Original proof of citizenship: This includes certified, original, or long form birth certificates; naturalization papers; certificate of citizenship; consular report of birth abroad; or a U.S. passport for applicants over the age of 16. Your original documents will be mailed to the Department of State and will be returned to you.  You must also bring a photocopy of your proof of citizenship.
  • Valid photo identification: Applicants and parents of applicants under age 16 must have a valid photo ID. You must also bring a photocopy of the front and back of this ID on one side of one piece of paper.
  • Current passport photo: At this time the library does not provide photo services. Please refer to the Department of State’s guidelines and bring your own photograph. Photographs must be 2×2, taken within the last 6 months, with a plain white background. No eyeglasses, uniforms, camouflage, or hats may be worn.

Please Note: Additional documentation is required for minors under the age of 16.  Both parents must be present.

Is There a Fee?

There are two fees, one payable to the Department of State and one payable to the Bayport-Blue Point Public Library (check or money orders only).

Please note, passport fees are determined by the Department of State:4

* Passport Card: For land or sea travel to Mexico, Canada, the Caribbean, and Bermuda. Cannot be used for air travel.

Optional Services & Fees

  1. Expedite fee – provides faster processing time (approx. 3 weeks), (optional) – $60 per application (Check/money order payable to U.S. Department of State)
  2. Express Mail of application to the U.S. Department of State (optional) – $24.70 (Check/money order payable to Bayport-Blue Point Public Library)
  3. Overnight delivery of passport book (optional) – $17.56 (Check/money order payable to U.S. Department of State)
  4. File Search Fee – applies when an applicant cannot present evidence of citizenship$150 (Check/money order payable to U.S. Department of State)

How Long Will it Take to Receive My Passport?

According to the U.S. Department of State, the processing times for passports are:

Routine:10-12 weeks

Expedited (extra charge): 4-6 weeks

Emergency Passport (life or death emergencies):  
If you are traveling within 2 weeks and need an emergency passport, the library cannot process the application.  You must make an appointment with the New York Passport Agency. You must show proof of travel (i.e. airline tickets).

How Do I Renew My Passport?

The library does not process passport renewals but we can review your application. To renew a 10-year adult passport, visit the State Department website and download form DS-82 along with the instructions. You can renew by mail if all of the following are true of your most recent U.S. passport:

  • It is undamaged and can be submitted with your application
  • It was issued when you were age 16 or older
  • It was issued within the last 15 years (not expired more than 5 years)
  • It was issued in your current name or you can legally document your name change with a certified copy of your marriage certificate or government-issued document evidencing your legal name change

Complete the application and mail it with payment and the old passport to the address on the form. If you were under 16 when your passport was issued (a 5-year passport), you cannot renew it. You must apply for a new passport.
